Original passport (must be 6 months valid beyond intended stay)
Original old passport if you have visa of the following (used within the last 5 years): Australia, Canada, Chile, Japan, Korea, Ireland, Israel, Mexico, New Zealand, Turkey, United Kingdom, US and Schengen
Original bank certificate with 6 months Average Daily Balance (ADB) addressed to the embassy
Photocopy of passbook
Photocopy of time deposit (if any)
Photocopy of last 3 months bank statement
Photocopy of last 3 months credit card statement
Photocopy of land title certificate
Photocopy of car registration OR/PR
Photocopy of any property documents (if any)
Photocopy of PSA marriage certificate (if traveling with spouse)
Photocopy of PSA birth certificate (if traveling with children)
Photocopy of PRC ID for professionals (Doctors, Engineers, Accountants etc.)
Photocopy of IBP ID for (lawyers)
Photocopy of alien certificate registration (ACR) for foreign passport
Photocopy of immigration certificate of residence(ICR) for foreign passport)
Daily travel itinerary
Round trip flight booking confirmation
Hotel booking confirmation

Original employment certificate with leave of absence (must include applicant’s position, date hired, salary, date of return to work)
Pay slips
Copy of PRC/IBP ID (if professional)
Copy of company ID
Copy of recent personal income tax return
